On actual wall and ceiling surface area, not floor area. As a rough rule, wall area is about 3 to 3.5 times the carpet area of a room.
No, and we will not quote it that way. Damp has to be traced and chemically treated first, otherwise the new paint will blister within a season. We quote the treatment separately so you can see the cost.
